Data Protection Notice
The Corner Dental Practice is the ‘controller’ for the personal data we process to provide you with dental care and treatment. The Corner Dental Practice complies with the General Data Protection Regulation and the Data Protection Act 2018.

When you register with the practice we will process personal information relating to you. This notice is for The Corner Dental Practice patients to advise them how we will process their personal data and how we comply with Data Protection Legislation. The Corner Dental Practice recognises the need to treat personal information in a fair and lawful manner.

What types of personal information do we process?
In order to provide you with safe and effective dental care and treatment we process the following personal information:
    ● Demographic information; name, age, address, telephone number(s)
    ● Past and current medical and dental conditions
    ● Records of dental care and treatment provided including X-ray images

Our purposes for processing your personal information
The Corner Dental Practice process personal information for the following purposes:
    ● To provide you with safe and effective dental care and treatment 
    ● To manage our service; including planning and auditing

Our lawful basis for processing personal information 
Current data protection legislation requires personal data to be processed in line with the following principles:
    ● Processed lawfully, fairly and in a transparent manner
    ● Collected for specified, explicit and legitimate purposes
    ● Adequate, relevant and limited to what is necessary
    ● Accurate and where necessary kept up to date
    ● Kept in a form which permits identification of data subjects for no longer than is necessary for the purposes for which these dates are processed
    ● Processed in a manner that ensures appropriate security of the personal data.  

The Corner Dental Practice will only process your personal information when the law allows us to. We will process your personal information in the following circumstances:
    ● As part of our official and public task in providing NHS Dental Services to you
    ● Where the processing is necessary for the purposes of the provision of health or social care or treatment
    ● As part of a contract between The Corner Dental Practice and you when providing private dental services

Who provides the personal information?
The personal information we process is collected directly from you when you register with us and from the records of the care and treatment provided to you.

Sharing of personal information
The Corner Dental Practice will share your personal information where this is lawful, necessary and appropriate with:
    ● Your GP
    ● Hospital or Community Dental Services where this is necessary for your care
    ● Another dentist or health professional who is caring for you
    ● NHS payments authorities
    ● The Benefits Agency, if you are claiming exemption or remission from NHS charges
    ● A private dental scheme, if you are a member 
    ● A laboratory

In these cases, only the minimum information required will be shared. In limited circumstances we may be required to share your personal data where it is required by law, e.g. a court order. Where possible we will inform you of the disclosure.

Security of your personal information
The Corner Dental Practice will take care to ensure your personal information is only accessible to authorised people. In general, access to your personal information is restricted to those who have a need to access it in order to carry out their legitimate duties.

Retaining personal information
We keep patient records for adult patients for a minimum of 11 years and for child patients for either 11 years or until the child is 25 years of age, whichever is longer.

The right of access
You have the right to access your own personal information. If you would like to access your personal information contained within your dental records please ask at reception or send your request in writing to The Practice Manager at the address below:

Your Duty to inform us of changes
It is very important that the personal information we hold about you is accurate and up to date. Please keep us informed if your personal information changes either in person, by telephone or in writing.

Complaints about how we process your personal information
If you are unhappy about how The Corner Dental Practice has processed your personal information, please contact: Mrs J. Wilson at The Corner Dental Practice (Tel. 01563 523053) and we will do our best to resolve the matter. If this fails, you can complain to the Information Commissioner at www.ico.org.uk/concerns or by calling 0303 123 1113.

Emergencies

During surgery opening hours
If you are a registered Private or NHS patient at the practice you will have access to our emergency appointment services.  

Please call the surgery as early as you can and we will aim to see you within 24 hours. We have dedicated appointment times every day to see and treat emergencies with priority given to the most urgent cases. We will also see patients with lost crowns and broken teeth as quickly as possible.

 Out of surgery hours   
If you have pain at the weekend or in the evening, you can contact NHS 24 on 111, who will give advice or arrange for you to be seen at one of their treatment centres. Details are on our answering machine  .
        
Some public holidays are covered by a local rota and the dentist on-call can be contacted via the mobile number given on our answering machine .
